A bunion is more than a bump beside the big toe. Also known as hallux valgus, it develops when the big toe gradually angles toward the smaller toes, making the joint at its base more prominent. The area may become red, swollen, or painful—particularly when it rubs against shoes—and the discomfort can eventually alter the way a person walks.
Bunions are very common. They affect one in three adults.
Early intervention and targeted exercise may help improve foot control and distribute pressure more evenly, Duke Pan, a physical therapist, told The Epoch Times. He recommends three exercises that can be practiced at home, particularly while the joint remains flexible.
Exercise cannot reliably reverse an established bony deformity, especially in advanced cases. However, strengthening the small muscles of the foot may improve stability, relieve symptoms, and help address movement patterns that place excessive stress on the big-toe joint.
As bunion pain worsens, people may unconsciously shift their weight away from the painful area. This compensation can change how the foot, ankle, knee, and hip work together. Restoring foot strength and a more balanced walking pattern is therefore an important part of conservative care—and may also support rehabilitation after surgery.
Why Bunions Develop
Bunions usually result from a combination of inherited foot structure and external stresses rather than a single cause.
Genetics and Foot Structure
Bunions often run in families. What is inherited is not necessarily the bunion itself, but structural characteristics—such as joint shape, ligament laxity, or the way the foot bears weight—that can make the big toe more likely to drift out of alignment.
People with a family history may therefore develop bunions earlier or with less external stress.
Tight or High-Heeled Shoes
High heels, particularly those with narrow or pointed toe boxes, shift more body weight toward the front of the foot and crowd the toes together. They may not be the sole cause of a bunion, but frequent use can aggravate an underlying structural tendency and worsen pain, redness, and friction.
Flat Feet or Collapsed Arches
A low or unstable arch can change how pressure travels through the foot. When the arch collapses excessively during walking, the foot may roll inward, placing additional stress on the joint at the base of the big toe.
This helps explain why some teenagers develop bunions even if they have never worn high heels.
Early signs may include swelling or enlargement at the base of the big toe, the toe beginning to angle inward, or recurring redness and friction when wearing shoes. People who notice these changes—especially those with a family history—may benefit from an evaluation by a physical therapist, podiatrist, or other qualified professional.

If you’re wondering if you have a genetic tendency toward bunions, you should first check your family history, Pan said. Early warning signs include swelling or enlargement at the base of the big toe, or redness and friction marks when wearing shoes. If these signs appear, it’s best to begin targeted exercise training as soon as possible.
Why do some people develop bunions after wearing high heels while others don’t, and why are dancers also at high risk for bunions?
Beyond genetics and foot structure, the decisive factor is whether the toes share the load evenly, Pan said. Those who frequently wear high heels, dancers, and people with flat feet or collapsed arches share one common trait: uneven pressure distribution across the foot.
In a normal walking pattern, force lands on the heel, travels along the outer edge of the foot, and then shifts inward to the big toe for the final push-off. When foot structure is compromised—high heels compress the forefoot and dance movements fail to spread pressure evenly across the ball of the foot—the highly mobile big toe ends up taking far more load than it should.
That’s why starting targeted foot-control exercises while the joint is still flexible and the soft tissues haven’t stiffened can restore proper muscle balance and weight distribution.
3 Exercises to Improve Foot Control
Early exercise training to restore proper foot control can significantly reduce the risk of a bunion worsening.
Exercise 1: Toe Separation Training
With a bunion, the big toe tends to drift toward the second toe and may even press against it. The goal of this exercise is to train the big toe to work independently, separate from the other toes, and press firmly and steadily into the ground.
Step 1: Support your body weight on your heels and slightly lift the toes, spreading all five as wide as possible.
Step 2: Keeping your toes spread, slowly press all five toes back down into the ground.
Step 3: You will feel a gentle activation along the inner side of the big toe and the inner arch muscles. Hold for about 10 seconds, then relax and repeat the movement.
The muscles along the inner arch primarily pull the big toe back into proper alignment. Consistent training of these muscles improves big-toe control, Pan said.
Exercise 2: Arch Training
Bunions tend to concentrate pressure on the joint at the base of the big toe on the inner side of the foot. Strengthening the arch helps distribute pressure more evenly.
Step 1: Spread all five toes and press them firmly into the ground. Then lift all five toes together, making sure they rise to the same height—avoid letting only the big toe lift much higher than the others.
Step 2: As the toes lift, you will feel the muscles of the inner arch activate—notice that sensation of engagement.
Step 3: Lower the toes back to the ground while maintaining the inner-arch activation—do not fully relax the muscles.
The foot muscles are very small, so it takes patience to truly feel them working, Pan said. Only with consistent, mindful practice will the benefits gradually accumulate.
Exercise 3: Calf Raise Training
Although rising onto the toes can contribute to bunions for many people, it can also be an effective corrective exercise when performed with proper form. The key is where you place your body weight.
Step 1: Stand with both feet firmly on the ground, then slowly rise onto the balls of the feet.
Step 2: Shift your weight so it rests between the second and third toes—avoid letting all the pressure fall on the big toe.
Step 3: As you rise, keep the ankles from tilting inward or outward. Keep checking that the pressure under your feet feels even, and adjust your center of gravity as needed.
Consistency Matters More Than Force
Foot retraining is subtle. The goal is not to force the big toe back into position, but to improve how the toes, arch, and ankle work together.
People with persistent pain, rapidly progressing deformity, numbness, difficulty walking, or diabetes should seek professional evaluation. More advanced bunions may require individualized footwear, orthotics, physical therapy, or—in severe cases—surgical correction.
